It must never happen again

It's an expression popular with politicians because they know that it's the message people want to believe. It didn't take me long to find an example on the web. Sam Potts is a Liberal Democrat councillor in the English town of Milton Keynes. On his website, he commented on the flooding that occurred in 2007:
"We need to look in depth at what went wrong here and do everything we can to make sure it doesn't happen again."
Not only does he invoke the zero-tolerance option, but he also uses the phrase make sure that is often used to reinforce the no uncertainty message.
